Beyond Game

TERMS & CONDITIONS: By submitting this form, the parent/guardian acknowledges and agrees to the following:

I give permission for my child to participate in the Beyond the Game Youth Leadership Program. I understand that participation involves travel and group activities.

As a participant, you agree your child can be photographed, videotaped and grant the Robbins Park District and Robbins CalPark Eagles permission to use the finished footage/photography for any purpose deemed appropriate, including but not limited to marketing, advertising, and public relations through print, radio, television, internet, and social media.

You release, discharge, and agree to hold harmless the Robbins Park District and Robbins CalPark Eagles, their affiliates, employees, officers, and directors from any liability related to the use of your child’s photograph, name, likeness, or performance, including claims for invasion of privacy, misappropriation, or defamation.

The Robbins Park District and Robbins CalPark Eagles reserve the right to deny participation to any individual or to remove any participant who fails to follow program rules and expectations.

All participants must comply with all rules and regulations established by the Robbins Park District and Robbins CalPark Eagles. Failure to comply may result in removal from the activity.

I fully release and hold harmless the Robbins Park District, Robbins CalPark Eagles, their officers, staff, employees, agents, and sponsors from any claims arising from injuries, illness, death, or loss of property related to participation.

I further acknowledge that the Robbins Park District and Robbins CalPark are not liable for events beyond their control, including but not limited to acts of God, weather conditions, accidents, criminal acts, civil disturbances, or other unforeseen circumstances.
