I understand that transportation and admission are included in the $5 fee. Cost of food and souvenirs are not included. I understand my child must arrive at the Robbins Park District at 13650 Claire Blvd, Robbins, IL 60472 by 9:00 am and the bus will be returning at 1:00pm.

TERMS & CONDITIONS: By submitting this form, the parent/guardian acknowledges and agrees to the following:

There is a $5 per person participation fee to attend the game. This fee includes your game ticket, transportation, and a special pre-game experience with the Chicago Sky. Payment must be received no later than July 15, 2026, by 3:30 PM. Please note that this fee is non-refundable. If you are unable to attend, you may transfer your reservation to another participant; however, the Robbins Park District must be notified in advance of any transfer and provided with the replacement participant’s information.

I give permission for my child to participate in the Robbins Park District trip to the Chicago Sky game. I understand that participation involves travel and group activities.

As a participant, you agree your child can be photographed, videotaped and grant the Robbins Park District and Robbins Friends of the Park permission to use the finished footage/photography for any purpose deemed appropriate, including but not limited to marketing, advertising, and public relations through print, radio, television, internet, and social media.

You release, discharge, and agree to hold harmless the Robbins Park District and Robbins Friends of the Park, their affiliates, employees, officers, and directors from any liability related to the use of your child’s photograph, name, likeness, or performance, including claims for invasion of privacy, misappropriation, or defamation.

The Robbins Park District and Robbins Friends of the Park reserve the right to deny participation to any individual or to remove any participant who fails to follow program rules and expectations.

All participants must comply with all rules and regulations established by the Robbins Park District and Robbins Friends of the Park. Failure to comply may result in removal from the activity.

I fully release and hold harmless the Robbins Park District, Robbins Friends of the Park, their officers, staff, employees, agents, and sponsors from any claims arising from injuries, illness, death, or loss of property related to participation.

I further acknowledge that the Robbins Park District and Robbins Friends of the Park are not liable for events beyond their control, including but not limited to acts of God, weather conditions, accidents, criminal acts, civil disturbances, or other unforeseen circumstances.
